Calibration Extension Sample Letter

Not sure if this is the correct area for this post.
I am looking for a calibration extension sample letter, if anyone would be willing to share.
What type of info would you consider necessary? (Other than the basic ID#, description, etc.)

We combine this type of request with our general change control form. We mainly require (1) customer rationale for the extension, and (2) a review of the calibration history of the instrument. Our SOP defines the default extension period that will be used once the request has been approved.

ISO says any amendments to the calibration certificate have to also have a statement "supplement to calibration certificate".

I'd try to keep it simple but would add the product line, location or lab where the instrument is used.
